Transparent guidelines for our services and the ICAN Support Program.

General Service Agreement

At Phendit Disability & Home Care, all ongoing supports are governed by a formal Service Agreement tailored to your specific needs and NDIS goals. This agreement is provided to you during the intake process and outlines:

  • The schedule of supports.
  • Responsibilities of the provider and the participant.
  • Budget and pricing (in line with the NDIS Price Guide).
  • Cancellation and termination policies.

Please refer to your signed Service Agreement for specific details regarding your personal care plan.

ICAN Support Program Terms & Conditions

The following terms apply specifically to participants accessing the Inclusive Care & Access Network (ICAN) program.

  1. Discretionary Access: ICAN supports are discretionary and subject to approval by Phendit management.
  2. Time-Limited: All ICAN supports are strictly time-limited and are not intended as long-term permanent solutions.
  3. Discount Limitations: Discounts (if applicable) do not apply to all service types and are assessed individually.
  4. Withdrawal of Support: ICAN supports may be withdrawn with notice if eligibility criteria are no longer met.
  5. No Lock-In Contracts: No lock-in service agreements apply to ICAN supports; you are free to leave at any time.
  6. Choice & Control: Participation in ICAN does not influence your choice or control over your NDIS plan or other providers.
  7. NDIS Compliance: All services delivered under ICAN strictly comply with NDIS Practice Standards.
  8. Transition: Participants may transition to standard Phendit services or another provider at any time.
  9. Future Funding: ICAN participation does not guarantee future funding outcomes from the NDIA.
  10. Integrity: Providing false or misleading information during the application may result in immediate withdrawal of support.
